About the role
Key responsibilities & impact- Perform case management telephonically and/or through home visits within the scope of licensure
- Use screening criteria knowledge and clinical judgment to assess member needs
- Conduct assessments to identify individual needs
- Develop care plans addressing assessment objectives and goals
- Monitor and evaluate care plan effectiveness and modify plans as needed
- Support member access to appropriate, quality, and cost-effective care
- Coordinate with internal and external resources to meet members’ identified needs
- Collaborate with providers
Requirements
What you’ll need- MA/MS in social work, counseling, or a related behavioral health field, or a degree in nursing
- Minimum of 3 years clinical experience in social work or counseling
- Broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment
- Current active unrestricted license such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, or Clinical Psychologist, as applicable by state law and scope of practice
- Must be licensed in the state of Georgia
- Must meet all applicable Georgia licensing and regulatory requirements to provide services to Georgia members
- For Government business only, LAPC or LAMFT may also be acceptable as allowed by applicable state laws
- Candidates must reside within commuting distance from a posting location unless accommodation is granted as required by law
- Willingness to work the specified day, evening, or overnight schedule
