About the role

Key responsibilities & impact

Perform care management activities within the scope of licensure for members with complex and chronic care needs, particularly oncology and REMS programs.
Ensure medically appropriate, high-quality, cost-effective care and appropriateness of treatment and level of care.
Partner with physicians and/or pharmacists to interpret appropriateness of care, plan interventions, and provide general clinical guidance.
Collaborate with providers to assess consumer needs and proactively identify adverse events and continuity-of-care needs.
Conduct clinical assessments to develop individualized goals and implement care plans.
Monitor and evaluate care-plan effectiveness and modify plans as necessary.
Collaborate with healthcare providers and/or pharmacists to drive personalized health management and improve consumer health outcomes.
Work as part of BioPlus Specialty Pharmacy, an Elevance Health company providing specialty medications, clinical expertise, digital capabilities, and personalized treatment support.

Requirements

What you’ll need

Requires an ASN or ADN and a minimum of 3 years of clinical experience; or any combination of education and experience, which would provide an equivalent background.
Current, valid, active unrestricted RN license in applicable state(s) required.
Multi-state licensure is required if providing services in multiple states.
BSN preferred.
Specialty Pharmacy experience preferred.
Bilingual preferred.
Drug Manufacturer experience preferred.
Telephonic Nursing experience preferred.
Must work Monday-Friday, 1:30pm–10pm EST.
May need to work holidays based on business need.
Must reside within a reasonable commuting distance from a posting location unless an accommodation is granted as required by law.
Certain patient/member-facing roles require COVID-19 and Influenza vaccination, subject to stated exceptions.
Ability to work in-office 1–2 days per week.
