About the role
Key responsibilities & impact- Provide exercise guidance to disease management members
- Collaborate with nurse case managers in managing disease management health goals of members
- Implement primary care physicians’ exercise recommendations
- Provide safe and effective exercise instruction and guidance
- Provide member health coaching, education, assessment and support of behavioral changes
- Promote self-management of chronic conditions
- Provide members with education materials related specifically to member score condition
Requirements
What you’ll need- BS/BA degree in exercise science, exercise physiology, kinesiology, physical therapy or physical education
- Minimum of 3 years experience as an exercise specialist, exercise physiologist or fitness instructor/trainer developing exercise prescriptions for special populations
- Any combination of education and experience which would provide an equivalent background
- Candidates must be within a reasonable commuting distance from the posting location(s), unless an accommodation is granted as required by law
- Minimum of 1 year cardiac rehab, pulmonary rehab, and physical therapy experience in clinical setting preferred
- MS in a related field preferred
- Membership in American Society of Exercise Physiologist preferred
- Experience working within a multi-disciplinary team preferred
- Candidates in certain patient/member-facing roles must become vaccinated against COVID-19 and Influenza
