About the role

Key responsibilities & impact
  • Conduct service coordination functions for a defined caseload of individuals in specialized programs
  • Facilitate the Person Centered Planning process with supported individuals
  • Conduct assessments and develop comprehensive Person Centered Support Plans and backup plans
  • Interface with Medical Directors and participate in interdisciplinary care rounds
  • Engage members’ support circles and manage physical health, behavioral health, and LTSS needs
  • Perform face-to-face program assessments using tools with predefined questions
  • Apply motivational interviewing techniques for evaluations, coordination, and management of waiver, behavioral health, and physical health needs
  • Identify potential clinical healthcare needs and gaps in care
  • Coordinate member cases as the single point of contact with clinical healthcare management and interdisciplinary teams
  • Manage non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities
  • Document short- and long-term service and support goals with members’ chosen care teams
  • Identify members who may benefit from alternative service levels or waiver programs
  • Mentor, serve as subject matter expert or preceptor, and assist with formal training of associates
  • Participate in process improvement initiatives
  • Submit utilization/authorization requests with documentation aligned to care plans
  • Report critical incidents to internal and external parties, including state and county agencies
  • Assist with and participate in appeals, fair hearings, member grievances, appeals, and state audits

Requirements

What you’ll need
  • BA/BS degree and a minimum of 2 years of experience working with a social work agency; or any combination of education and experience which would provide an equivalent background
  • Specific education, years, and type of experience may be required based upon state law and contract requirements
  • Candidate must reside in Charlotte, Collier, Desoto, Hendry, Glades, Lee, or Sarasota County
  • Ability to work Monday-Friday, 8am-5pm EST
  • Bilingual in Spanish/English
  • Experience with LTSS, case management, social work, or hospital discharge planning preferred
  • BA/BS degree in a health care related field preferred
  • Candidates must be within a reasonable commuting distance from the posting location(s), unless an accommodation is granted as required by law
  • New candidates in certain patient/member-facing roles must become vaccinated against COVID-19 and Influenza, unless an acceptable explanation is provided
