About the role

Key responsibilities & impact
  • Deliver high-quality, trauma-informed, culturally competent, person-centered coordination for MyCare Ohio members
  • Manage members' LTSS cases
  • Perform telephonic or face-to-face assessments within the scope of licensure
  • Identify, evaluate, coordinate, and manage members' physical health, behavioral health, social services, and long-term services and supports needs
  • Identify members at risk for complications and obtain clinical data
  • Identify members who may benefit from alternative levels of care or waiver programs
  • Provide collected information to the RN for verification, interpretation, additional assessment, and care-plan development
  • Coordinate care for members with chronic illnesses, co-morbidities, and/or disabilities under the direction of the responsible RN
  • Collaborate with the RN, member, and healthcare team to support cost-effective and efficient utilization of health benefits
  • Apply disease-process terminology and clinical guidelines to member needs without requiring nursing judgment
  • Travel to client sites or designated locations, with occasional office attendance for meetings or training

Requirements

What you’ll need
  • Candidates should be located in Butler, Warren, Clinton, Clermont, Hamilton, Montgomery, Lucas, Williams, Defiance, Paulding, Fulton, Henry, Wood, Ottawa, Sandusky, Erie, Darke, or Miami counties in Ohio
  • Requires an LPN/LVN, LSW, LCSW, LMSW, or license other than RN in accordance with applicable state law
  • Nursing Diploma or Associate of Science in Nursing or a related field, or equivalent combination of education and experience
  • Minimum of 2 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role
  • Current, unrestricted applicable LPN/LVN, LSW, LCSW, LMSW, or other non-RN license required
  • Strong preference for case management experience with older adults or individuals with disabilities
  • BA/BS in Health/Nursing preferred
  • Must comply with applicable vaccination requirements for certain patient/member-facing roles, unless an acceptable explanation is provided
  • Must be within a reasonable commuting distance from the posting location(s), unless an accommodation is granted as required by law
