About the role
Key responsibilities & impact- Deliver high-quality, trauma-informed, culturally competent, person-centered coordination for MyCare Ohio members
- Assist the responsible RN with telephonic or face-to-face assessments
- Identify, evaluate, coordinate and manage members’ physical health, behavioral health, social services and long-term services and supports needs
- Identify members at high risk for complications and obtain clinical data as directed
- Identify members who may benefit from an alternative level of care or waiver programs
- Provide collected information to the responsible RN for verification, interpretation, additional assessment and care-plan development
- Participate in coordinating care for members with chronic illnesses, co-morbidities and/or disabilities
- Coordinate with the RN, member and healthcare team to support cost-effective and efficient utilization of health benefits
Requirements
What you’ll need- Must reside in the state of Ohio
- Requires an LPN/LVN, LSW, LCSW, LMSW or license other than RN in accordance with applicable state law
- Nursing Diploma or AS in Nursing or a related field, and minimum of 2 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role; or equivalent combination of education and experience
- Current, unrestricted applicable LPN/LVN, LSW, LCSW, LMSW or other non-RN license required
- Strong preference for case management experience with older adults or individuals with disabilities
- BA/BS in Health/Nursing preferred
- Candidates in patient/member-facing roles may be required to become vaccinated against COVID-19 and Influenza
