About the role

Key responsibilities & impact

Manage members' cases within the scope of RN licensure
Provide direction and oversight to non-RN clinicians participating in members' cases
Develop, monitor, evaluate, and revise member care plans
Perform telephonic and face-to-face clinical assessments
Identify and coordinate care for members at high risk of complications
Manage members with chronic illnesses, co-morbidities, and/or disabilities
Obtain member histories and develop individualized care plans
Establish short- and long-term goals with members, caregivers, families, natural supports, and physicians
Identify members who may benefit from alternative levels of care or waiver programs
Ensure members' access to care-plan services
Facilitate service authorizations and referrals when appropriate
Interface with Medical Directors, Physician Advisors, and interdisciplinary teams on care-management treatment plans
Assist with provider, claims, and service issue resolution
Assign tasks to non-RN clinicians, verify and interpret member information, conduct additional assessments, and review their performance

Requirements

What you’ll need

HS diploma or equivalent
Minimum of 3 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role; or any combination of education and experience providing an equivalent background
Current, active, valid, and unrestricted RN license in applicable state(s) required
May require state-specified certification based on state law and/or contract
Associates/MA/MS in Health/Nursing preferred
Candidate should reside in Passaic County, NJ
Must be within a reasonable commuting distance from the posting location unless an accommodation is granted
Travel to worksite and other locations as necessary
Candidates in certain patient/member-facing roles must become vaccinated against COVID-19 and Influenza, unless an acceptable explanation is provided
