About the role

Key responsibilities & impact
  • Manage members' cases within the scope of RN licensure
  • Develop, monitor, evaluate, and revise person-centered care plans
  • Perform telephonic and face-to-face functional assessments
  • Identify, evaluate, coordinate, and manage members' physical health, behavioral health, social services, and long-term services and supports needs
  • Identify members at high risk for complications and coordinate care with members and healthcare teams
  • Manage members with chronic illnesses, co-morbidities, and/or disabilities to support cost-effective utilization of health benefits
  • Obtain thorough and accurate member histories
  • Establish short- and long-term goals with members, caregivers, families, natural supports, and physicians
  • Identify members who may benefit from alternative levels of care or waiver programs
  • Ensure members' access to required services
  • Facilitate authorizations and referrals for services when appropriate
  • Interface with Medical Directors, Physician Advisors, and interdisciplinary teams on person-centered care plans
  • Assist with provider, claims, or service issue problem-solving
  • Travel to client sites or designated locations, with occasional office attendance for meetings or training

Requirements

What you’ll need
  • Must reside in the state of Ohio
  • High school diploma or GED equivalent
  • Minimum of 3 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role
  • Current, active, valid, and unrestricted RN license in Ohio required
  • Strong preference for case management experience with older adults or individuals with disabilities
  • BA/BS in Health/Nursing preferred
  • Ability to work within a reasonable commuting distance from the posting location(s), unless an accommodation is granted as required by law
  • Candidates in patient/member-facing roles may be required to become vaccinated against COVID-19 and Influenza
