About the role

Key responsibilities & impact
  • Collaborate with company physicians, patients’ other physicians and providers, and family members to develop complex plans of care
  • Provide clinical and non-clinical support to patients
  • Provide urgent and primary care through telephone and tele-video modalities
  • Develop and implement clinical plans of care for adult patients with chronic and complex conditions
  • Gather patient histories, perform physical exams, obtain diagnostics as needed, and develop treatment plans
  • Identify and close gaps in care
  • Address patients’ and families’ physical and psychosocial needs with interdisciplinary team support
  • Educate patients and families about medications, side effects, illness progression, diet and nutrition, adherence, and crisis prevention
  • Coordinate care and resources with clinical team members, physicians, and medical providers
  • Maintain patient medical records and documentation in accordance with regulations and company standards
  • Participate in continuing education as required by state and certifying body
  • Prescribe medication as permitted by state prescribing authority

Requirements

What you’ll need
  • MS in Nursing
  • Active, national NP certification
  • Valid, current, active, and unrestricted Family or Adult Nurse Practitioner license in IA, IN, KS, NJ, OH, TN, TX, VA, or WA
  • Valid, current, active RN Compact license
  • 2+ years of experience managing complex care cases
  • Experience working with Electronic Medical Records (EMR)
  • DEA registration or eligibility preferred
  • Experience managing diabetes, behavioral health, and serious illness, including palliative care, highly preferred
  • Active Medicaid certification preferred
  • Experience managing complex care cases for developmental disabilities and chronically ill patients
  • Candidates must reside within a reasonable commuting distance from the posting location(s), unless accommodation is granted as required by law
  • Ability to work Monday–Friday, 8:00 am–5:00 pm CST or EST, with rotating on-call
