About the role

Key responsibilities & impact
  • Collaborate with company physicians, patients’ other physicians and providers, and family members to develop complex plans of care
  • Provide clinical and non-clinical support to patients
  • Provide primary and urgent healthcare via telephone and tele-video modalities
  • Serve patients receiving home and community-based services through state Medicaid programs, dual eligible members, and other assigned members
  • Develop and implement clinical plans of care for adult patients with chronic and complex conditions
  • Gather patient history, physical examination findings, and diagnostics as needed
  • Develop and implement treatment plans based on patient goals and current conditions
  • Identify and close gaps in care
  • Address patients’ and families’ physical and psychosocial needs with interdisciplinary team support
  • Educate patients and families about medication usage, side effects, illness progression, diet and nutrition, medical adherence, and crisis anticipation and prevention
  • Coordinate care and resources with clinical team members, physicians, medical providers, patients, and families
  • Maintain patient medical records and documentation according to regulations and company standards
  • Participate in required continuing education
  • Prescribe medication as permitted by state authority

Requirements

What you’ll need
  • Experienced Nurse Practitioner candidate
  • Active, unrestricted Family or Adult Nurse Practitioner license in the State of Tennessee
  • Active RN Compact license
  • MS in Nursing
  • Active, national NP certification
  • Valid, current, active and unrestricted Family or Adult Nurse Practitioner (NP) license in the state of Tennessee
  • Valid, current, active RN Compact license
  • 2+ years of experience in managing complex care cases
  • Experience working with Electronic Medical Records (EMR)
  • Possession of DEA registration or eligibility preferred
  • Active Tennessee Medicaid license highly preferred
  • Experience managing complex care cases for developmental disabilities and chronically ill patients strongly preferred
  • Candidates must be within a reasonable commuting distance from the posting location(s), unless an accommodation is granted as required by law
  • New candidates in certain patient/member-facing roles must become vaccinated against COVID-19 and Influenza, unless an acceptable explanation is provided
