About the role
Key responsibilities & impact- Provide primary and urgent healthcare by telephone and tele-video to patients receiving home and community-based services
- Develop and implement clinical care plans for adults with chronic and complex conditions
- Gather medical histories, conduct physical examinations, obtain diagnostics as needed, and create treatment plans
- Identify and close gaps in care
- Address patients’ and families’ physical and psychosocial needs with interdisciplinary team support
- Educate patients and families about medications, side effects, illness progression, diet, nutrition, adherence, and crisis prevention
- Coordinate care and resources with company physicians, other physicians, medical providers, patients, families, and clinical team members
- Maintain patient medical records and documentation in accordance with regulations and company standards
- Participate in required continuing education
- Prescribe medication as permitted by state authority
- Provide clinical and non-clinical patient support
Requirements
What you’ll need- Bilingual Nurse Practitioner licensed in the state of New York
- Active RN license
- MS in Nursing
- Active national NP certification
- Valid, current, active, and unrestricted Family or Adult Nurse Practitioner license in New York
- Experience working with Electronic Medical Records (EMR)
- 2+ years of experience managing complex care cases
- Bilingual or multi-lingual skills in Spanish, Vietnamese, Mandarin, and/or Russian
- Active RN Compact license preferred
- Experience managing complex care cases for developmental disabilities and chronically ill patients strongly preferred
- DEA registration or eligibility preferred
- Bilingual Spanish preferred
- Active Medicaid number in New York highly preferred
- Must be within reasonable commuting distance of the posting location unless accommodation is granted, despite the primarily virtual arrangement
- Must work Monday–Friday, 8:00 a.m.–5:00 p.m. EST and rotating on-call 2–3 days per month, 5:00 p.m.–8:00 a.m. EST
- Certain patient/member-facing roles require COVID-19 and Influenza vaccination, subject to stated exceptions
