Utilize community, government, and/or client resources to address participant limitations and support chronic-condition management
Manage behavioral and psychosocial needs to improve clinical and financial outcomes
Deliver social work interventions
Help members use available resources to meet personal health needs and develop their capabilities
Evaluate members’ ability to manage independently and locate alternative resources through Social Work Psychosocial evaluations
Guide members seeking solutions to social, cultural, or financial problems affecting healthcare management
Evaluate members’ health self-management strengths
Develop strategies and implement plans supporting case decisions
Facilitate and coordinate behavioral health resources based on individual member needs

Requirements

What you’ll need

Requires MS (at a minimum) in Social Work
Minimum of 3 years of experience in case management in a health care environment
Current unrestricted LMSW or LCSW (or equivalent) license in applicable state(s) required
Bilingual (Spanish) or multi-language skills preferred
Clinical Social Work experience
Strong technical proficiency
Candidates must reside within a reasonable commuting distance from the posting location(s), unless accommodation is granted as required by law
