About the role
Key responsibilities & impact- Perform case management telephonically and/or through home visits within scope of licensure for special programs such as Foster Care
- Manage overall healthcare costs for the designated population through integrated physical health and behavioral health case management and whole-person health
- Conduct assessments to identify individual needs
- Develop comprehensive care plans addressing assessment objectives and goals
- Support member access to appropriate, quality, cost-effective care and modify plans as needed
- Coordinate internal and external resources to meet members’ integrated physical, behavioral, and whole-person care needs
- Coordinate social determinants of health and incorporate them into care planning
- Work closely with state agencies
- Maintain knowledge of the system-of-care philosophy and coordinated community-based services and supports
- Build partnerships with designated populations and families while addressing cultural and linguistic needs
- Evaluate health needs and identify applicable services and resources with members and families
- Provide patient education and medication reconciliation
- Identify community resources and assist with arranging follow-up care
- Travel within the assigned region for facility or home visits
- Attend the office occasionally for meetings or training and work at an Elevance Health location at least once per week under the company’s hybrid workforce strategy unless the role is specified as primarily virtual
Requirements
What you’ll need- MS/MA in social work, counseling, or a related behavioral health field, or a degree in nursing
- Minimum of 3 years of clinical experience in social work counseling with broad experience in complex psychiatric and substance abuse or substance abuse disorder treatment, or an equivalent combination of education and experience
- Active, current, and valid license such as LCSW, LMSW, LPC, LAPC, LMFT, LMHC, or RN issued by the Commonwealth of Virginia
- Experience working with specialty populations preferred
- Case management experience with complex psychiatric/substance abuse and/or medical disorders strongly preferred
- Knowledge of Virginia Foster Care strongly preferred
- Prior experience working with Community Services Board (CSB) and/or Department of Social Services (DSS) preferred
- Computer literacy
- Experience using Microsoft Word, Excel, and Outlook preferred
- Ability to travel to worksites and other locations when necessary
- Candidates must reside within a reasonable commuting distance from the posting location(s), unless an accommodation is granted as required by law
- Ability to work Monday–Friday, 8:00AM–5:00PM Eastern Time
- COVID-19 and Influenza vaccination may be required for certain patient/member-facing roles
