About the role

Key responsibilities & impact
  • Perform care management activities within the scope of licensure for members with complex and chronic care needs, particularly oncology and REMS programs
  • Ensure medically appropriate, high-quality, cost-effective care and treatment appropriateness
  • Partner with physicians and/or pharmacists on care appropriateness, intervention planning, and clinical guidance
  • Collaborate with providers to identify consumer needs, adverse events, and continuity-of-care requirements
  • Conduct clinical assessments and develop individualized goals and care plans
  • Monitor and evaluate care-management plan effectiveness and modify plans as needed
  • Collaborate with healthcare providers and pharmacists to drive personalized health management and improve consumer health outcomes

Requirements

What you’ll need
  • ASN or ADN and a minimum of 3 years of clinical experience; or any combination of education and experience providing an equivalent background
  • Current, valid, active unrestricted RN license in applicable state(s)
  • Multi-state licensure if providing services in multiple states
  • Oncology experience preferred
  • BSN preferred
  • Specialty Pharmacy experience preferred
  • Bilingual skills preferred
  • Drug Manufacturer experience preferred
  • Telephonic Nursing experience preferred
  • Ability to work Monday-Friday 12:30-9 EST or 1:30-10pm EST
  • May need to work holidays based on business need
  • Must reside within a reasonable commuting distance of a posting location unless an accommodation is granted
  • COVID-19 and Influenza vaccination required for certain patient/member-facing roles
