About the role
Key responsibilities & impact- Provide health coaching services to disease management program members
- Deliver education, motivational support, and individualized action planning
- Provide information to members eligible for disease management programs
- Conduct telephonic meetings with program members according to program standards
- De-escalate situations with members resistant to health coaching or disease management interventions
- Consult department associates on methods to help members recognize and overcome health barriers
- Increase participants’ motivation by addressing preventative care, barriers, and needs
- Promote incremental behavior change and healthier outcomes
- Conduct behavioral assessments to identify individual knowledge, skills, and behavioral needs
- Coordinate with the multidisciplinary clinical team as needed
Requirements
What you’ll need- BA/BS in appropriate field of specialization
- Minimum of 2 years of related experience in direct care health education, exercise instruction, or patient education
- Any combination of education and experience providing an equivalent background may be considered
- Prior experience in health coaching, disease management in a call center environment, and/or patient education and behavior change techniques preferred
- Strong understanding of disease management principles preferred
- Certification or advanced certification and/or license appropriate to field of specialty preferred
- Prior experience utilizing Motivational Interviewing techniques preferred
- Candidates must reside within a reasonable commuting distance from a posting location unless an accommodation is granted as required by law
- New candidates in certain patient/member-facing roles must become vaccinated against COVID-19 and Influenza, unless an acceptable explanation is provided
- Ability to work Monday–Friday, 1:00 pm to 9:30 pm EST
