About the role

Key responsibilities & impact
  • Plan, organize, and direct home health and hospice care
  • Complete initial, comprehensive, and ongoing patient and family assessments
  • Provide physical assessments, nursing care, medications, and treatments according to the physician’s plan of care
  • Develop and revise individualized care plans and establish patient goals
  • Initiate preventive and rehabilitative nursing procedures
  • Counsel and educate patients and families on health care, medications, diet, self-care, and related needs
  • Assist patients with activities of daily living and support self-sufficiency and comfort care
  • Act as case manager and coordinate care for an assigned caseload
  • Maintain accurate clinical notes and document pain and symptom management
  • Communicate with physicians, community health professionals, patients, families, and caregivers
  • Work with the interdisciplinary group and attend interdisciplinary group meetings
  • Participate in on-call duties
  • Ensure required equipment, services, and supplies are available
  • Supervise ancillary personnel and delegate responsibilities when required
  • Participate in continuing education, quality assessment, and performance improvement activities

Requirements

What you’ll need
  • Graduate of National League for Nursing accredited school of nursing
  • Minimum of two years of experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred
  • Registered nurse with current licensure to practice professional nursing in the state
  • Demonstrates excellent observation, problem solving, verbal and written communications; nursing skills per competency checklist
  • Shows ability to organize and prioritize workload independently
  • Able to lift, position, and/or transfer patients
  • Able to lift supplies and equipment
  • Visual acuity and hearing to perform required nursing skills
  • Maintains a current CPR certification
  • Must have and maintain a valid driver’s license, maintain automobile insurance coverage and have access to an automobile
  • Self-directed and able to work with minimal supervision
