About the role

Key responsibilities & impact
  • Monitor patient status remotely using telehealth technology and electronic health record systems
  • Identify changes in patient condition and facilitate clinical escalation and intervention
  • Support bedside clinicians through real-time clinical consultation and workflow guidance
  • Coordinate communication among physicians, nurses, case managers, and other care-team members
  • Assist with evidence-based practices, quality initiatives, and patient safety protocols
  • Document clinical activities and recommendations in electronic health record systems
  • Participate in multidisciplinary rounds and care coordination across partnered healthcare facilities
  • Serve as a clinical resource for hospitals, emergency departments, intensive care units, step-down units, post-acute settings, and home-based care programs
  • Provide centralized clinical support to hospitals and healthcare partners nationwide
  • Perform proactive patient monitoring, clinical escalation, workflow optimization, and collaboration with on-site teams through Equum's virtual care platform

Requirements

What you’ll need
  • Active unencumbered Compact RN license
  • Active NY license required
  • Current BLS certification required
  • Current ACLS certification required
  • Minimum five (5) years of acute care nursing experience
  • Experience in ICU, step-down, emergency, or progressive care environments strongly preferred
  • Previous experience as a Charge Nurse, Preceptor, Team Lead, Clinical Coordinator, or similar leadership role preferred
  • Experience utilizing electronic medical record systems such as Epic, Cerner, or Meditech
  • Comfortable working in a fast-paced telehealth and technology-driven environment
