About the role
Key responsibilities & impact- Enroll patients in IBCCP
- Track bills, budget, and caseload; enter information into Cornerstone
- Provide case management to IBCCP enrollees diagnosed with cancer
- Work with an RN to ensure oversight
- Provide patient navigation for high-risk and newly diagnosed cancer patients, primarily breast cancer and potentially colorectal and cervical cancer patients
- Help patients understand diagnoses, treatment options, and internal and external resources
- Maintain current knowledge of community resources
- Coordinate support groups and events, including two monthly breast cancer support groups, two yearly mobile salons, and one yearly cervical and breast cancer event
- Perform other duties as assigned
Requirements
What you’ll need- High School diploma or equivalent required
- Minimum of A.A/A.S in Community Health or Health Education, LPN, MA, or equivalent preferred
- Bilingual (English/Spanish) required
- At least 2 years in a healthcare setting
- Proficiency using MS Word, Excel, and Outlook
- Experience using electronic medical records
- Familiarity with Cornerstone is a plus
- Experience with special populations, especially cancer care population, preferred, or strong desire to learn this population in a short period of time
