About the role

Key responsibilities & impact
  • Provide direct patient care in outpatient and nursing home facilities
  • Assess patients referred by Primary Care Providers using evidence-based assessment tools
  • Treat behavioral health conditions using CBT, Motivational Interviewing, crisis intervention, Interpersonal Psychotherapy, and behavioral activation
  • Make treatment recommendations during collaborative care team meetings
  • Monitor treatment through care planning and coordination with clinical teams and health homes
  • Provide crisis intervention services and risk assessments as needed
  • Engage clients with limited behavioral health service utilization through outreach efforts
  • Document services in a timely manner
  • Provide clinical supervision and/or consultation in the absence of the Clinical Manager
  • Participate in semi-monthly clinical supervision, team meetings, and case conferences
  • Provide field instruction to MSW students when appropriate

Requirements

What you’ll need
  • LCSW licensure
  • Master of Social Work degree from an accredited institution
  • Ability to utilize behavioral health screening tools and perform biopsychosocial assessments
  • Ability to utilize electronic health records for documentation of clinical services and review of relevant health data
  • Ability to communicate clinical information succinctly and comprehensively to integrated care teams, including PCP, NP/MD, and Health Home Care Management staff
  • Bilingual English/Spanish preferred
