About the role
Key responsibilities & impact- Serve as the primary point of contact for clients in the ECM program
- Coordinate care across medical, behavioral health, and social service providers
- Conduct client strengths, needs, and goals assessments through interviews, screenings, and evaluations
- Develop individualized case plans collaboratively with clients
- Provide one-to-one education, life skills training, workshops, and group activities
- Adapt training methods and materials to diverse learning styles, cultural backgrounds, and developmental levels
- Help clients set achievable life skills goals and monitor progress
- Empower clients in care decisions and educate them about rights, resources, and self-advocacy
- Connect clients with service providers, agencies, community resources, support networks, vocational programs, education classes, and social services
- Maintain knowledge of available resources and collaborate with community partners
- Provide emotional support, active listening, validation, problem-solving assistance, and crisis-resource access
- Maintain accurate client records, case plans, case notes, and documentation
- Prepare and submit reports on client progress, service utilization, and outcomes
- Ensure compliance with confidentiality regulations and agency policies
- Collaborate with interdisciplinary teams and participate in case consultations and multidisciplinary meetings
- Perform other duties and projects as assigned
Requirements
What you’ll need- High School diploma or equivalent required
- Associate or Bachelor’s degree in Social Work, Psychology, or related field preferred
- One (1) year of direct experience in a Behavioral Health or Social Services setting preferred
- Computer literacy with proficiency in Microsoft Office Suite and knowledge of database and reporting tools
- Excellent verbal, written, and interpersonal communication skills
- Excellent time management, organizational, and multitasking skills
- High level of accuracy and attention to detail
- Ability to maintain discretion and confidentiality with sensitive information
- Frequent driving is an essential function of the job
- Must possess a valid California driver's license and maintain an acceptable driving record
- Must have a minimum of two (3) years of licensed driving experience
- Must be eligible for coverage under the organization's motor vehicle insurance policy
- Flexibility to meet client needs and accommodate required meetings, supervision, and program-related responsibilities, including possible weekends, holidays, split shifts, and evenings

Comp & perks- Spanish Bilingual differential - 10% increase on the employee's hourly base rate of pay (if applicable)
- Mileage reimbursement $0.76 cents per mile
