About the role
Key responsibilities & impact- Perform initial client evaluations
- Develop and revise Service Plans/Care Plans and physicians’ orders, when applicable
- Conduct supervisory visits in clients’ residences
- Oversee client services and ensure client needs are met
- Conduct annual evaluations and competencies
- Provide and coordinate services according to the Service Plan/Care Plan
- Initiate preventive and rehabilitative non-medical procedures
- Document client assessments, care plans, supervisory visits, caregiver and client education, and training
- Inform personnel of changes in client condition and needs
- Implement the nursing care plan, when applicable
- Observe, record, and report signs, symptoms, and reactions to non-medical treatments
- Manage limited skilled nursing services, when applicable
- Counsel clients and families regarding service needs
- Participate in quality assurance reviews and agency service evaluations
- Coordinate staff meetings, client care conferences, and in-services
- Take on-call duty, including nights, weekends, and holidays
- Supervise and teach team members
- Assist clients with appropriate self-care techniques
- Perform other assigned activities
Requirements
What you’ll need- Graduate of a state-approved school of nursing
- Currently licensed as a Registered Nurse in the state(s) in which practicing
- Minimum of two (2) years’ in-home care experience required
- Minimum of one (1) year supervisory experience preferred
- Proficient in Microsoft Office and/or Google Suite and scheduling software
- Excellent organizational, communication, interpersonal, and leadership skills
- Current state Registered Nurse (RN) licensure
- Current driver’s license and auto insurance
- Ability to read and understand short instructions, correspondence, and memos
- Ability to effectively communicate through written and verbal language
- Ability to read, analyze, and interpret business periodicals, professional journals, technical procedures, or governmental regulations
- Ability to write reports and business correspondence
- Ability to present information and respond to questions from managers, clients, customers, and the general public
- Ability to evaluate situations and make complex decisions using critical thinking and reasoning
- Ability to deal effectively with high levels of stress
