About the role

Key responsibilities & impact
  • Manage referrals and schedule patients in a timely manner
  • Perform patient evaluations
  • Document communication notes for evaluations not completed within specified timeframes
  • Supplement the virtual caseload with in-person house calls, SLC visits, and other patient-care tasks as needed
  • Perform and document patient virtual screenings
  • Coordinate with regional clinicians for patient transitions, care coordination, and evaluation shadowing
  • Apply virtual programs, including tests and measures, interventions, and patient/outcome tracking
  • Participate in virtual care marketing
  • Participate in telehealth teaching or training for field clinicians
  • Attend recurring meetings with leadership and Virtual Care Team members
  • Participate in practice initiatives, sales, and marketing efforts
  • Meet contract quota expectations bi-weekly
  • Maintain KPI standards and comply with FOX, federal, state, and payer regulations
  • Maintain professional attire and communicate effectively with team members and seniors

Requirements

What you’ll need
  • Must be licensed as a PT
  • PT licensed in Connecticut required
  • Ideal candidate would live in a PT compact state
  • Must meet all educational/credentials requirements of a Physical Therapist
  • Must be willing to obtain and maintain licensure in other states outside of their home state as determined appropriate by leadership
  • Must have experience treating the older adult population
  • Must have 1 year of telehealth experience
  • Must be willing to provide in-person house calls and/or SLC visits as needed
