About the role
Key responsibilities & impact- Manage referrals and schedule patients in a timely manner
- Perform patient evaluations
- Document communication notes for evaluations not completed within specified timeframes
- Supplement the virtual caseload with in-person house calls, SLC visits, and other patient-care tasks as needed
- Perform and document patient virtual screenings
- Coordinate with regional clinicians for patient transitions, care coordination, and evaluation shadowing
- Apply all aspects of virtual programs, including tests and measures, interventions, and patient/outcome tracking
- Participate in virtual-care marketing
- Participate in telehealth teaching or training for field clinicians
- Participate in recurring meetings with leadership and Virtual Care Team members
- Participate in practice initiatives, sales, and marketing efforts
- Meet contract quota expectations bi-weekly
- Maintain KPIs and comply with FOX, federal, state, and payer regulations, policies, and procedures
- Maintain professional attire and a neat appearance
- Communicate effectively with team members and seniors
Requirements
What you’ll need- Must be licensed as a SLP
- SLP license in New Jersey required
- ASHA Certification required
- Ideal candidate would live in a SLP compact state
- Must meet all educational/credentials requirements of a Speech Language Pathologist
- Must be willing to obtain and maintain licensure in other states outside of their home state as determined appropriate by leadership
- Must have experience treating the older adult population
- Must have 1 year of telehealth experience
- Must be willing to provide in-person house calls and/or SLC visits as needed
