About the role

Key responsibilities & impact
  • Provide psychosocial services to in-center and home dialysis patients
  • Assess patients’ psychosocial status, strengths, and areas of need
  • Participate in care planning with patients and the healthcare team
  • Provide supportive, educational, and goal-directed counseling
  • Support patients adjusting to dialysis and achieving optimal psychosocial status and quality of life
  • Refer patients to community, vocational, educational, home health, and financial resources
  • Assess advance directives and support informed care decisions
  • Participate in discussions of patient DNR status
  • Document social work interactions and interventions
  • Provide psychosocial support for non-adherence, quality outcomes, and quality-of-life concerns
  • Participate in monthly quality review meetings and report quality indicators
  • Educate patients and families about kidney disease, ESRD, treatment modalities, rights, responsibilities, and support services
  • Facilitate transplant referrals and collaborate on transplant waitlist management
  • Support patient admission, continuity of care, scheduling, and discharge-request assessment
  • Liaise with Insurance Coordinators regarding insurance, payment, coverage changes, and financial assistance
  • Refer patients to the patient billing solutions department
  • Assist with personnel interviews and hiring decisions when requested
  • Maintain patient resource lists
  • Train staff on psychosocial topics as needed
  • Participate in weekly team huddles and discuss urgent patient issues
  • Perform other related duties as assigned

Requirements

What you’ll need
  • Masters in Social Work
  • Must have state required license
  • Must meet applicable scope of practice board and licensure requirements in the state of employment
  • 0–2 years’ related experience
  • Travel required if multiple facilities or home visits, if applicable
