About the role

Key responsibilities & impact
  • Provide psychosocial services to in-center and home dialysis patients
  • Assess patients’ psychosocial status, strengths, and needs affecting rehabilitation and treatment outcomes
  • Participate in interdisciplinary care planning and identify effective interventions
  • Provide monitoring, supportive counseling, education, and goal-directed counseling
  • Assist patients with community, vocational, educational rehabilitation, and advance directive resources
  • Participate in DNR discussions with physicians, nurses, patients, and families
  • Document MSW interactions and interventions
  • Provide psychosocial support for non-adherence, quality outcomes, and quality-of-life concerns
  • Participate in monthly quality review meetings and report adherence and quality indicators
  • Educate patients and families about kidney disease, ESRD, treatment modalities, rights, responsibilities, and support services
  • Facilitate transplant referrals and collaborate on transplant waitlist management
  • Support patient admission, scheduling, continuity of care, and discharge-request assessments
  • Liaise with insurance coordinators regarding coverage, payment, and financial assistance
  • Refer billing questions to the patient billing solutions department
  • Support staff interviews and hiring when requested
  • Maintain patient resource lists and train staff on psychosocial topics
  • Participate in weekly team huddles and discuss urgent patient issues
  • Float between North Greeley and Loveland clinics

Requirements

What you’ll need
  • Masters in Social Work
  • Must have state required license
  • 2 – 5 years’ related experience
  • Knowledge of social work theory and accepted methods of social work practice
  • Ability to provide supportive counseling within the scope of clinical training and state license
  • Ability to assess psychosocial status and participate in patient care planning
  • Knowledge of kidney disease, dialysis, ESRD psychosocial issues, and support services
  • Ability to facilitate transplant referrals and collaborate with interdisciplinary teams
  • Knowledge of insurance, financial assistance, federal/state/commercial insurance, state renal programs, AKF HIPP, and entitlement programs
  • Ability to travel between multiple facilities when required
