About the role
Key responsibilities & impact- Provide psychosocial services to in-center and home dialysis patients
- Assess patients’ psychosocial status, strengths, and needs affecting rehabilitation and treatment outcomes
- Participate in care planning and develop interventions with patients and the healthcare team
- Monitor and support patients adjusting to dialysis and improving psychosocial status and quality of life
- Provide supportive, educational, and goal-directed counseling, including transplant counseling
- Refer patients to community, vocational, educational, home health, and rehabilitation resources
- Assist with advance directive and DNR discussions with patients, families, physicians, and nurses
- Document social work interactions and interventions
- Provide psychosocial support for adherence, quality outcomes, and quality-of-life concerns
- Participate in monthly quality review meetings and report adherence and quality indicators
- Educate patients and families about kidney disease, treatment modalities, ESRD psychosocial issues, rights, responsibilities, and support services
- Facilitate transplant referrals and collaborate on transplant waitlist management
- Support patient admission, continuity of care, scheduling, and discharge-request investigations
- Liaise with Insurance Coordinators regarding coverage, payment, renal programs, AKF HIPP, and entitlement programs
- Refer billing questions to the patient billing solutions department
- Assist with hiring when requested, maintain patient resource lists, train staff, and participate in weekly team huddles
- Perform other related duties as assigned
Requirements
What you’ll need- Master's in Social Work
- Must have state required license
- Meets the applicable scope of practice board and licensure requirements in effect in the State in which they are employed
- 0–2 years’ related experience
- Travel required if multiple facilities or home visits, if applicable
- Ability to provide supportive counseling within the scope of clinical training and state license
- Knowledge of social work theory, methods, and psychosocial interventions
- Knowledge of dialysis, Chronic Kidney Disease, ESRD, transplant processes, insurance, financial assistance, and community resources
- Ability to document patient and family interactions and interventions
- Ability to collaborate with interdisciplinary healthcare teams
- Ability to provide patient and staff education and participate in quality improvement activities
