About the role
Key responsibilities & impact- Provide nutritional services for in-center and home patients to maximize nutritional status and improve clinical outcomes
- Assess patients’ knowledge of diet and kidney disease and provide individualized education
- Educate patients, families and caregivers on nutritional status, adequacy measures, fluid status and nutrition-related laboratory parameters
- Complete nutritional assessments and participate in interdisciplinary comprehensive assessments
- Develop individualized comprehensive plans of care with the interdisciplinary team
- Calculate diet prescriptions according to standards of practice
- Complete progress notes and maintain accurate medical-record documentation
- Provide ongoing nutrition assessment and outcome-oriented nutrition counseling
- Identify malnourished or at-risk patients and implement appropriate interventions
- Monitor adherence and response to nutrition therapy and adjust therapy as needed
- Collaborate on Urea Kinetic Modeling, mineral metabolism, renal bone disease, anemia, iron status, psychosocial status, vascular access, modality, rehabilitation and sodium/volume control
- Report nutrition QAI results and participate in quality improvement activities
- Coordinate Pre-ESRD education and patient education plans
- Review nutrition-related laboratory results, counsel patients, recommend treatment changes and communicate results to physicians and care providers
- Participate in staff and care plan meetings
- Consult with the FMS Corporate Dietitian as needed
- Maintain and improve professional knowledge and skills
- Collaborate with the interdisciplinary team to ensure work areas are safe and clean
- Perform other nutrition management and related duties as assigned
Requirements
What you’ll need- Registered Dietitian as per Commission on Dietetic Registration
- Current state licensure if applicable
- Minimum of 1-year professional work experience in clinical nutrition as a Registered Dietitian
- Previous renal experience preferred
- Demonstrated leadership competencies and skills, including ability to teach, strong organizational skills, excellent communication, customer service, relationship development, results orientation, team building and decision making
- Able to perform responsibilities with minimum supervision
- Basic computer skills required
- Maintains dietetic registration and continuing education hours as specified by American Dietetic Association and state licensure regulations where applicable
- Ability to travel between assigned facilities and various locations within the community
- May be exposed to infectious and contagious diseases/materials
