About the role

Key responsibilities & impact
  • Assess patients in their place of residence or in hospitals and prepare patients for entry into Gilchrist Hospice Care services
  • Serve as an ambassador for Gilchrist Hospice Care within the service area
  • Complete initial assessments of patients and families/caregivers to determine hospice eligibility and needs
  • Prepare and implement initial care plans with Medical Director approval
  • Provide direct nursing care to maintain the highest level of care
  • Inform the Team Leader about utilization of team members, outside agencies, and community resources
  • Document clinical findings to support hospice eligibility and participate in discharge planning
  • Teach family members, caregivers, and patients care skills including pain control, medication, injections, blood draws, IV management, dressing changes, hygiene, nutrition, and vital-sign checks
  • Arrange necessary home equipment
  • Attend to patients and families at the time of death when occurring during a visit
  • Participate in Admissions department projects and process improvement strategies
  • Establish and maintain relationships with hospice referral sources and meet with healthcare administrators in the community
  • Obtain necessary consents according to applicable regulations and reimbursement policies

Requirements

What you’ll need
  • Diploma, ADN and/or BSN from an accredited institution
  • Current MD nursing license
  • Three years of nursing experience
  • Knowledge of universal precautions for infection control and safe nursing standards for self-protection
  • Strong time management skills and ability to function with a changing daily schedule
  • Excellent interpersonal and communication skills
  • Ability to be patient and family focused and provide patient/caregiver education
  • Skill in using EMR (Electronic Medical Record)
