About the role
Key responsibilities & impact- Work with individuals over age 3 with developmental disabilities and their families
- Advocate for and secure services to improve clients’ lives
- Participate in assessments of individuals and family needs
- Develop, implement, monitor and coordinate Individual Program Plans
- Complete required documentation and reports
- Manage a wide range of active cases
- Participate in ongoing social assessments and overall case planning
- Collect information for interdisciplinary teams
- Analyze data and identify barriers to developmental objectives
- Initiate and terminate regional center funding
- Ensure purchased services are provided
- Maintain records in compliance with federal and state requirements
- Advocate for receipt of entitled services
- Help clients and families coordinate and use available resources
- Review and ensure effective services from community resources
- Initiate referrals and monitor vendored-service progress
- Evaluate service availability and effectiveness
- Identify community service gaps and assist in developing needed services
- Provide outreach services as assigned
- Participate in internal and community committees and task forces
Requirements
What you’ll need- Bachelor’s Degree from an accredited school of social welfare or related human services field
- Master’s Degree preferred
- Fluency in Spanish, including colloquial and relevant technical/functional terminology and relevant spoken/written dialects
- Knowledge of principles, objectives and techniques of counseling individuals
- Knowledge of normal and abnormal individual and group behavior, human growth and development of family relationships
- Knowledge of community structure and organization and how to intervene constructively on client's behalf
- Ability to work independently, exercise judgment, and cooperate as part of an interdisciplinary team
- Excellent communication skills and ability to convey complex information clearly, concisely and professionally
- Ability to stimulate cooperation of clients, families, community agencies or individuals
- Ability to establish and maintain effective working relationships
- Knowledge of identifying and navigating generic services
- Knowledge of typical and atypical child development behavior
- Case Management experience strongly desired
- Access to reliable transportation regularly to visit programs, often without advanced notice
