About the role

Key responsibilities & impact
  • Provide training, advocacy, service arrangements and administrative support for Self Determination Program participants
  • Monitor the Self Determination Program and report progress according to program guidelines
  • Provide internal consultation on SDP to the ongoing social work unit
  • Collaborate with and support GGRC’s Self Determination Local Advisory Committee, fiscal unit and resource development units
  • Serve as the main internal and external point of contact for SDP questions and training
  • Respond to communications from individuals, families, service providers and outside agencies
  • Develop program plans with clients, families and other stakeholders and monitor results
  • Provide advocacy services and train individuals and families on advocacy issues
  • Maintain case, funding and billing records and produce required reports
  • Conduct person-centered planning meetings and develop Individual Program Plans and individual budgets
  • Serve as primary GGRC staff support and partner to the SDLAC and attend monthly meetings
  • Complete monitoring requirements and progress reports, including reports to DDS
  • Complete SDP budget funding and related reports
  • Potentially serve as an Independent Facilitator, coordinating and monitoring service delivery and progress
  • Provide community information and advocacy, including maintaining GGRC’s Self Determination webpage
  • Respond to SDP@ggrc.org community inquiries
  • Serve as backup to Social Workers as needed
  • Participate voluntarily in the Supervisory On-Call rotation
  • Participate in special projects and perform additional assigned duties
  • Maintain confidentiality in compliance with HIPAA and the California Lanterman Act

Requirements

What you’ll need
  • Bachelor’s Degree in Social Work or related human services field
  • Valid Clinical License at time of hire and ability to maintain license in good standing preferred
  • 4+ years post-Master’s work with increasing responsibility
  • 2+ years Regional Center Experience
  • Demonstrated knowledge of agency policy and procedures
  • Training and facilitation skills
  • Interview techniques for person-centered planning
  • Knowledge of counseling principles, objectives and techniques
  • Knowledge of individual and group behavior, human growth and development, and family relationships
  • Knowledge of community structure and organization and constructive client advocacy
  • Ability to maintain clear and concise case records
  • Strong written and oral communication skills
  • Ability to operate basic office equipment
  • Basic familiarity with MS Word, MS Excel, MS PowerPoint, MS Outlook or similar software
  • Ability to provide direction focused on client outcomes and values
  • Planning and project management skills
  • Ability to coach and support staff and provide real-time feedback
  • Creative problem-solving skills
  • Ability to address performance and hold people accountable
