About the role

Key responsibilities & impact

Conduct behavioral and care assessments in patients' homes or approved alternate settings
Complete face-to-face Start of Care, reassessment, and PRN visits
Assess IADL mobility, nutrition, and environment
Identify active and chronic diseases and determine physical, mental, and social needs
Educate and counsel patients and families on identified conditions
Develop and implement service plans and regularly reassess client needs
Observe and report symptoms, treatment reactions, drug reactions, and changes in physical or emotional condition
Initiate preventative procedures and environmental changes to ensure patient safety and security
Maintain clinical and progress notes and provide progress reports to physicians
Coordinate services with other agencies and healthcare providers
Counsel patients and families regarding nursing and related needs
Complete documentation electronically using a paperless system
Attend virtual training through Zoom

Requirements

What you’ll need

Graduate of an accredited school of nursing
Valid and current RN license
CPR certified
1-3 years of nursing experience; home care experience preferred
In-home care new client assessment experience preferred
Supervisory skills a plus
Excellent communication skills
Flexible and able to interact positively with staff
Strong sense of initiative and urgency in addressing unexpected patient crises
Self-directed and able to work without an immediate supervisor
Commitment to regulatory compliance and timely documentation
Valid state driver license and automobile insurance
Strong computer skills, including Word, Excel, EMRs, and web-based applications
Strong attention to detail
Able to complete assessments and corrections in a timely manner
Able to complete and submit assessments within 24 hours
Respond to clarification phone calls within 24 hours
Make reminder calls 24 or 48 hours before scheduled appointments
Must have a tablet, phone, or laptop and be comfortable with technology
Comfortable sending and receiving email, using a scheduling platform, and completing electronic assessments in a web browser
Must provide an updated resume, RN license and registration, Social Security Card or proof of citizenship, and state-issued ID before Zoom training
