About the role
Key responsibilities & impact- Triage inbound sick calls from Medical Group patients
- Assess patient symptoms and provide appropriate medical advice
- Determine the level of care required and coordinate care with the care team
- Review clinical test results and summarize findings to assist providers
- Prioritize and respond to inbound phone calls, voicemails, and test results
- Assess patients’ symptoms and medical history over the phone using probing questions and the EMR
- Use evidence-based guidelines to determine urgency and assign triage priority
- Identify crisis situations and connect patients with emergency services according to protocol
- Educate patients, caregivers, and others on health maintenance, disease prevention, self-care, medications, and follow-up steps
- Make outbound calls to communicate provider orders
- Collaborate with care teams to complete patient follow-ups and promote quality outcomes
- Communicate with patients and families, addressing concerns and providing emotional support
- Analyze test-result data and communicate needs, patterns, and variances to providers
- Enter patient results and quality metrics according to protocol
- Maintain compassionate written and verbal communication with care team members
- Adhere to company policies, quality standards, and productivity expectations
- Identify, acknowledge, escalate, and address patient complaints, grievances, and concerns
- Participate in assigned projects and quality improvement initiatives
- Perform additional duties assigned by the Clinical Triage Manager and/or Director
Requirements
What you’ll need- Graduate of approved or accredited nursing education program as a Registered Nurse
- Current valid RN license
- Must be able to obtain and maintain RN license within all service states within 6 months from hire
- Satisfactory completion of required nursing continued education requirements
- Ability to react decisively and quickly in urgent and emergent situations
- Strong comprehension of medical terminology and pharmaceuticals
- Demonstrated experience with multi-tasking
- Strong communication skills (written & spoken)
- Demonstrated proficiency in using electronic health records (HER) system
- Ability to communicate effectively with providers, patient care teams, patients, patients’ family members and facility staff
- Ability to work remotely and independently
- Knowledge of basic principles, practices, and techniques in primary healthcare
- Strong organizational skills and attention to detail
- Conflict resolution skills
- Familiarity with Medicare and Medicaid insurances and programs (preferred)
- 2+ years of experience as an RN (preferred)
- Contact center experience (preferred)
- Nurse triage experience (preferred)
- Experience in Aprima (EHR) (preferred)
- Multistate License (MSL) (preferred)
- Candidates may be required to complete a background check in the state of Florida
