About the role

Key responsibilities & impact
  • Deliver annual risk assessments in residential settings or via telehealth
  • Conduct comprehensive in-home health risk assessments
  • Identify active and chronic disease conditions and physical, mental, and social needs
  • Take patient histories, perform examinations, and determine diagnoses
  • Document patient visits according to NCQA standards
  • Take patient vital signs as necessary
  • Place case management referrals and communicate with PCPs
  • Communicate with patients, caregivers, agency nurses, providers, and vendors
  • Perform clinical duties while observing OSHA Universal Precautions
  • Maintain patient confidentiality
  • Attend required meetings and in-services and participate in committees
  • Participate in professional development activities and maintain professional licenses and affiliations

Requirements

What you’ll need
  • Candidates must reside within Sussex County, NJ
  • Active/unrestricted nurse practitioner license to practice in coverage states
  • Board certification through ANCC, AANP, or NCCPA
  • Active BLS Certification
  • Current enrollment in Medicare/Medicaid
  • Valid driver’s license and good driving record
  • Outstanding EHR skills
  • Geriatric training/experience preferred
  • Ability to handle/lift patients and regularly lift/carry items weighing up to 50 pounds
  • Ability to ride in an automobile or van up to 150 miles daily
  • Ability to drive, if necessary
