About the role
Key responsibilities & impact- Deliver annual risk assessments in residential settings or via telehealth within the Nurse Practitioner scope of practice
- Conduct comprehensive in-home health risk assessments to identify active and chronic disease conditions and patients’ physical, mental, and social needs
- Take patient histories, perform examinations, and determine diagnoses
- Provide written documentation of patient visits according to NCQA standards
- Take patient vital signs as necessary
- Place case management referrals and communicate with primary care providers
- Communicate with patients, caregivers, agency nurses, other providers, and vendors to ensure proper diagnosis
- Perform clinical duties while observing OSHA Universal Precautions
- Maintain patient confidentiality
- Attend required meetings and in-services and participate in committees as requested
- Participate in professional development activities and maintain professional licenses and affiliations
- Perform other job-related assignments as requested
- Collaborate with teammates, physicians, medical staff, patients, caregivers, agency nurses, providers, and vendors
Requirements
What you’ll need- Must be located in Minden, NV
- Active/unrestricted nurse practitioner license to practice in coverage states
- Board certification through ANCC, AANP, or NCCPA
- Active BLS Certification
- Current enrollment in Medicare/Medicaid
- Valid and unencumbered driver's license with good driving record
- Outstanding EHR skills
- Ability to adapt to differing weather conditions and patients’ home/residential environments
- Full range of body motion, including handling/lifting patients
- Ability to regularly lift/carry items weighing up to 50 pounds
- Ability to ride in an automobile or van up to 150 miles daily in urban and/or rural settings
- Ability to drive, if necessary
- Geriatric training/experience preferred
