About the role

Key responsibilities & impact
  • Deliver annual health risk assessments in residential settings or via telehealth
  • Conduct comprehensive in-home health risk assessments to identify active and chronic disease conditions and determine physical, mental, and social needs
  • Take patient histories, perform examinations, and determine diagnoses
  • Provide written documentation of patient visits according to NCQA standards
  • Take patient vital signs as necessary
  • Place case management referrals and communicate with PCPs as necessary
  • Communicate with patients, caregivers, agency nurses, other providers, and vendors to assure proper diagnosis
  • Perform clinical duties while observing OSHA Universal Precautions
  • Maintain patient confidentiality
  • Attend required meetings and in-services and participate in committees as requested
  • Participate in professional development activities and maintain professional licenses and affiliations
  • Perform other job-related assignments as requested

Requirements

What you’ll need
  • Active/unrestricted nurse practitioner license to practice in coverage states
  • Board certification in one of the following: American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP) or National Commission on Certification of Physician Assistants (NCCPA)
  • Active BLS Certification
  • Current enrollment in Medicare/Medicaid
  • Valid and unencumbered driver's license with good driving record
  • Outstanding EHR skills
  • Geriatric training/experience preferred
  • Ability to handle differing weather conditions and patients’ home/residential environments
  • Full range of body motion including handling/lifting patients; standing, bending, walking and stair climbing
  • Ability to regularly lift/carry items weighing up to 50 pounds
  • Ability to ride in an automobile or van up to 150 miles daily in urban and/or rural settings
  • Ability to drive, if necessary
