About the role
Key responsibilities & impact- Assist care managers in managing the overall health of enrolled populations
- Improve recognition, treatment, and management of psychosocial, behavioral, and medical conditions
- Provide clinical mental health consultation services within the scope of state professional licensure
- Participate in management of psychosocial aspects of chronic and acute diseases
- Apply behavioral principles to lifestyle and health-risk issues
- Consult and co-manage treatment of mental disorders and psychosocial issues
- Provide mental health and substance abuse assessment, intervention, and connection to higher levels of care
- Develop clinical pathways and best-practice programs for targeted patient groups
- Participate in staff meetings, trainings, daily huddles, and system-wide BHC learning collaboratives
- Educate and support multidisciplinary team members during patient treatment planning
- Provide population-based care to high-risk and high-need patients
- Promote mental health and substance abuse change efforts within the practice and health system
- Document care in the shared medical record
- Engage patients by telephone and in person
- Provide same-day assessment, brief therapeutic counseling, interventions, referrals, and follow-up
- Use solution-focused approaches, CBT, Motivational Interviewing, and other short-term counseling interventions
- Coordinate with families and support systems with patient permission
- Provide crisis support, triage, and referrals to specialty mental health and substance abuse services
- Advocate for patients and re-engage patients who are not participating in care
- Connect patients with community resources related to social determinants of health
- Coach patients to actively participate in their care and treatment
Requirements
What you’ll need- Master’s degree in Mental Health Field
- 2 years experience in use of evidence-based counseling techniques, screening, diagnosis, and treatment of common mental health and substance abuse disorders
- Knowledge of the relationship between medical and psychological aspects of health and disease
- Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) or Licensed Marriage and Family Therapist (LMFT)
- For out-of-state license transfers, transfer license within 60 days of hire
- CPR – American Heart Association
- Act 34 Criminal Background Clearance Certificate
- Act 33 Child Abuse Clearance Certificate
- Act 73 FBI Fingerprinting Criminal Background Clearance Certificate
- Compliance with HIPAA, privacy policies, information security guidelines, code of business conduct, and applicable laws
