About the role
Key responsibilities & impact- Act as a core healthcare team member assisting care managers with the overall health of enrolled populations
- Develop clinical pathways and best-practice programs for targeted patient groups
- Participate in staff meetings, trainings, daily huddles, system-wide BHC learning collaboratives, and population health efforts
- Provide behavioral health, substance abuse, and psychosocial education and support to multidisciplinary team members
- Provide population-based care to high-risk/high-need patients and all enrollees
- Promote change efforts around mental health and substance abuse disorders
- Document in the shared medical record
- Engage patients by telephone and in person
- Provide same-day assessment, brief therapeutic counseling, interventions, and referrals to higher levels of care
- Use solution-focused approaches, CBT, motivational interviewing, and other short-term counseling interventions
- Coordinate with families and support systems with patient permission
- Provide follow-up and relapse-prevention education
- Promote skill building and facilitate groups when appropriate
- Address urgent mental health and substance abuse needs, triage, and refer to specialty services
- Advocate for patients, re-engage nonparticipating patients, encourage active care participation, and connect patients with community resources related to social determinants of health
Requirements
What you’ll need- Master’s degree in Mental Health Field
- 2 years experience in use of evidence-based counseling techniques, screening, diagnosis, and treatment of common mental health and substance abuse disorders
- Knowledge of the relationship between medical and psychological aspects of health and disease
- Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) or Licensed Marriage and Family Therapists (LMFT); out-of-state license transfers allowed within 60 days of hire
- CPR – American Heart Association
- Act 34 Criminal Background Clearance Certificate
- Act 33 Child Abuse Clearance Certificate
- Act 73 FBI Fingerprinting Criminal Background Clearance Certificate
- Compliance with HIPAA, data security guidelines, company policies, applicable laws, and training requirements
