About the role

Key responsibilities & impact
  • Assist care managers in managing the overall health of enrolled populations
  • Improve recognition, treatment, and management of psychosocial, behavioral, and medical conditions
  • Provide clinical mental health consultation services within the scope of professional licensure
  • Participate in management of psychosocial aspects of chronic and acute diseases and apply behavioral principles to lifestyle and health-risk issues
  • Provide mental health and substance abuse assessment, intervention, and connection to higher levels of care
  • Develop clinical pathways and best-practice programs for targeted patient groups
  • Participate in practice staff meetings, trainings, daily huddles, and system-wide BHC learning collaborative
  • Provide behavioral health and substance abuse education and support to multidisciplinary team members
  • Deliver population-based care to high-risk and high-need patients
  • Document in the shared medical record
  • Engage patients by telephone and in person
  • Provide same-day assessment, intervention, brief counseling, referrals, and crisis support
  • Use solution-focused approaches, CBT, Motivational Interviewing, and other short-term counseling interventions
  • Identify functional outcomes and recommend interventions for occupational, social, familial, home, and recreational functioning
  • Coordinate with family members and support systems with patient permission
  • Provide follow-up and relapse-prevention education
  • Promote psychological and physical health skill building, including groups when appropriate
  • Triage and refer patients to specialty mental health and substance abuse services
  • Advocate for patients, re-engage patients who are not participating in care, encourage active treatment participation, connect patients with community resources, and provide coaching

Requirements

What you’ll need
  • Master’s degree in Mental Health Field
  • 2 years experience in use of evidence-based counseling techniques, screening, diagnosis, and treatment of common mental health and substance abuse disorders
  • Knowledge of the relationship between medical and psychological aspects of health and disease
  • Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) or Licensed Marriage and Family Therapist (LMFT)
  • For out of state license transfers, transfer license within 60 days of hire
  • CPR – American Heart Association
  • Act 34 Criminal Background Clearance Certificate
  • Act 33 Child Abuse Clearance Certificate
  • Act 73 FBI Fingerprinting Criminal Background Clearance Certificate
  • Compliance with HIPAA, data security guidelines, Code of Business Conduct, applicable laws, regulations, company policies, and training requirements
