About the role
Key responsibilities & impact- Assess patient need for diabetes education
- Provide individualized diabetes self-management education and training based on assessment
- Collect DSME/T program participant assessment data using a recognized certification database/program
- Develop educational goals, learning objectives, educational content, and teaching methods with program participants
- Develop individualized follow-up plans with program participants
- Evaluate educational services by measuring attainment of learning objectives
- Conduct follow-up assessments using outcome measures from the AADE7 Continuum of Outcomes Framework
- Document assessment data, educational plans, services provided, and evaluation results in participant educational records
- Participate in developing education curricula and materials
- Plan and conduct continuing education programs for professional staff and the community
- Perform other duties as assigned or required
Requirements
What you’ll need- Bachelor’s Degree
- Registered/Licensed Dietitian
- Experience in counseling patients with diabetes
- Experience in community education
- Certified Diabetes Care and Education Specialist (CDCES) or eligible and must obtain CDCES certification within nine months of hire
- Act 34 Criminal Background Clearance Certificate
- Act 33 Child Abuse Clearance Certificate
- Act 73 FBI Fingerprinting Criminal Background Clearance Certificate
- Compliance with HIPAA, data security guidelines, the Code of Business Conduct, applicable laws, regulations, company policies, and training requirements
