About the role
Key responsibilities & impact- Visit Medicaid members in their homes, Assisted Living Facilities, Long Term Care Facilities, and other care settings
- Travel locally 75–90% of the time
- Assess and evaluate members’ needs and requirements
- Establish member-specific care plans
- Ensure members receive services in the least restrictive setting
- Plan and implement interventions
- Coordinate services and monitor and evaluate case management plans against members’ personal goals
- Guide members and families toward appropriate care resources
- Facilitate interactions with payer sources, providers, interdisciplinary teams, and others involved in members’ care
- Report to the Regional Care Coach Manager
- Work Monday–Friday, 8:30 a.m.–5:00 p.m.
Requirements
What you’ll need- Bachelor's Degree with at least 2 years of relevant experience in Health Services, Social Work, or Psychology
- Registered Nurse (RN), licensed in the state of Florida, with at least 2 years of relevant experience
- Licensed Practical Nurse (LPN), licensed in the state of Florida, with at least 4 years of relevant experience
- Case Worker with a minimum of 6 years of Health Services/Case Management Experience
- Prior experience with Medicare & Medicaid recipients
- Intermediate to advanced computer skills
- Experience with Microsoft Office programs
- AHCA Fingerprint Required
- Required to complete tuberculosis (TB) screening
- Valid state driver's license
- Proof of personal vehicle liability insurance
- Prior nursing home diversion or long-term care case management experience
- Bilingual English/Spanish with ability to pass a Language Proficiency Exam
- Experience with electronic case note documentation and multiple computer applications/systems
- Experience working with geriatric population
- Experience with health promotion, coaching and wellness
- Knowledge of community health and social service agencies and additional community resources
- 701B Certification
- Medication application (RFA) experience
- Subject to Florida Level 2 background screening through the Care Provider Background Screening Clearinghouse
