About the role
Key responsibilities & impact- Evaluate members' needs and requirements to achieve or maintain optimal wellness
- Guide members and families toward appropriate care and wellbeing resources
- Coordinate services and resources to support members' wellness
- Identify and resolve barriers to effective care
- Monitor patient care and progress toward desired outcomes using assessments, data, conversations, and active care planning

Requirements
What you’ll need- Bilingual English and Spanish, Polish, Arabic, Creole, or another language
- Bachelor's degree in a health and human services field OR LPN with the ability to be licensed in Illinois
- 2 or more years of healthcare experience
- Knowledge of Microsoft Office Word and Excel
- Exceptional verbal, written communication, and interpersonal skills
- Ability to use a variety of electronic information applications/software programs
- Passion for contributing to an organization focused on continuously improving consumer experiences
- Separate room with a locked door suitable for a home office to ensure continuous privacy
- Minimum home internet speed of 25 Mbps download and 10 Mbps upload
- Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information
