About the role

Key responsibilities & impact

Manage members' health issues using telephonic strategies, approaches and techniques
Identify and resolve barriers that hinder effective care
Conduct comprehensive assessments and continuously monitor patient care and progress toward desired outcomes
Develop and use active care plans based on assessments, data and conversations with members
Guide members and their families to essential healthcare and community resources
Follow professional concepts, standards, regulations, strategies, operating standards and established procedures
Make decisions regarding work approach and priorities under managerial direction
Report to the Manager, Care Management

Requirements

What you’ll need

Bachelor's degree in a health-related field, active Illinois LPN licensure, or ability to obtain an Illinois LPN license
2 or more years of case management, medical case management, care coordination, or healthcare support experience
Comprehensive knowledge of Microsoft Office applications, including Word, Excel, and Outlook
Bilingual English/Spanish preferred
Certified Case Manager (CCM) preferred
Master's Degree preferred
Managed care experience preferred
Prior experience with Medicare and Medicaid recipients preferred
Experience with electronic case note documentation and multiple computer applications/systems preferred
Experience with health promotion, coaching and wellness preferred
Knowledge of community health and social service agencies and community resources preferred
