About the role

Key responsibilities & impact
  • Assess and evaluate members' needs and requirements in a telephonic behavioral health environment
  • Guide members and families toward appropriate care and wellbeing resources
  • Manage members' physical, environmental and psychosocial health issues using varied strategies and techniques
  • Identify and resolve barriers that hinder effective care
  • Continuously monitor patient care through assessments and evaluations
  • Ensure patients progress toward desired outcomes
  • Create member care plans as needed
  • Make independent decisions regarding work methods and courses of action
  • Report to the Regional CM Manager
  • Travel up to 50% of the time for mandatory field work
  • Occasionally travel to Humana offices for training or meetings

Requirements

What you’ll need
  • Must have an active, unrestricted SC Licensed Masters Social Worker (LMSW), Licensed Professional Counselor (LPC) OR an active, unrestricted compact Registered Nurse (RN) license
  • 2 or more years of experience working as a behavioral health professional
  • Reside in the state of South Carolina and willing to travel
  • Have a separate room with a locked door that can be used as a home office to ensure continuous privacy while you work
  • Experience with behavioral change, health promotion, coaching and wellness
  • Proficient in using electronic information systems and Microsoft Office tools (Word, Outlook, Excel)
  • Experience supporting patients telephonically (preferred)
  • Experience working in Community Mental Health, crisis response teams, Assertive Community Treatment (ACT) teams, and substance use disorder treatment (preferred)
  • Experience working with both children and adults (preferred)
  • Must be able to work a 40 hour work week, Monday through Friday, 8:00 AM to 5:00 PM
  • Must meet minimum home internet speeds of 25 Mbps download and 10 Mbps upload
  • Overtime may be requested to meet business needs and requires approval
