About the role

Key responsibilities & impact

Provide comprehensive social service care management for frail elders and adults with intellectual, developmental, or physical disabilities in Wisconsin's Family Care program
Learn what is important to members and support them in achieving it
Partner with a Health and Wellness Coordinator (RN) and collaborate on member needs and care plans
Perform face-to-face social-related assessments upon enrollment and at least every six months thereafter in the member's setting
Coordinate, perform, and track quarterly face-to-face visits and monthly phone contacts
Participate in development, ongoing review, and coordination of members' plans of care
Lead social, community integration, employment, housing, and non-health-related issues
Educate members on and promote prevention and wellness and mitigate assessed risks
Document activities and complete required paperwork
Travel throughout Jefferson County and provide services at business partner facilities or members' homes

Requirements

What you’ll need

Four-year bachelor's degree in Human Services or a related field and one (1) year of experience working with at least one Family Care target population, OR a four-year bachelor's degree in any other field and three (3) years' related experience working with at least one Family Care target population
Experience working with frail elders and/or adults with intellectual, developmental, or physical disabilities
Valid state driver's license
Proof of personal vehicle liability insurance with at least 100/300/100 limits
Must participate in Humana's driver safety program
Must undergo tuberculosis (TB) screening if selected
Ability to travel up to 40% throughout Jefferson County, Wisconsin
Work Monday–Friday, 8:00 am–4:30 pm CST
Home internet service with at least 25 Mbps download and 10 Mbps upload for Home or Hybrid Home/Office employees
Ability to work from a dedicated space without ongoing interruptions to protect member PHI/HIPAA information
