About the role
Key responsibilities & impact- Provide comprehensive case management services to frail elders and adults with intellectual, developmental, or physical disabilities in the Wisconsin Family Care program
- Assess members' health and safety needs, strengths, interests, and preferences to develop comprehensive Member Care Plans
- Coordinate with a Field Care Nurse to address members' health and safety needs in the least restrictive environment
- Conduct face-to-face social assessments upon enrollment and at least every six months, typically at members' residences
- Conduct quarterly in-person visits and maintain monthly telephone contact with members
- Arrange support services involving social integration, community resources, employment, housing, and other non-medical needs
- Ensure cost-effective service delivery
- Evaluate risk factors and provide education to members
- Maintain accurate case notes, service authorizations, and Member Care Plan updates
- Travel up to 40% within the Ashland/Superior coverage area
- Report to the Manager of Care Coaching
Requirements
What you’ll need- Bachelor's degree in human services or a related field and at least 1 year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities; alternatively, a Bachelor's degree in another field and at least 3 years of such experience
- Valid state driver's license
- Proof of personal vehicle liability insurance with at least 100/300/100 limits
- Reside within 45 minutes or less of the assigned coverage area (preferred)
- Case Management experience (preferred)
- Experience with electronic case note documentation (preferred)
- Knowledge of community health and social service agencies and additional community resources (preferred)
- Ability to meet work-at-home internet requirements: minimum 25 Mbps download and 10 Mbps upload
- Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information
- Ability to work Monday–Friday, 8:00 am–4:30 pm CST
- Willingness to participate in Humana's TB screening program
