About the role
Key responsibilities & impact- Build and maintain relationships with community-based organizations, schools, faith-based groups, coalitions, and local stakeholders
- Represent the organization at community events, health fairs, meetings, coalitions, conferences, and stakeholder forums
- Identify collaboration opportunities aligned with organizational priorities, health outcome goals, and population health strategies
- Serve as liaison between internal teams and external partners to address community needs and improve access to services
- Support outreach strategies that increase brand awareness and member retention
- Apply public health principles and social drivers of health to identify barriers affecting members and communities
- Monitor community trends, disparities, and emerging needs to inform outreach priorities and organizational strategy
- Create and manage a partnership portfolio supporting referral pathways for members
- Manage budgeted funds and resources effectively
- Document outreach activities, events, referrals, and partnership engagement in QuickBase or other tracking systems
- Maintain accurate data entry for record and reporting integrity
- Travel throughout the assigned region for events, presentations, coalition meetings, and outreach activities
- Maintain working knowledge of plan benefits and the Medicaid landscape
- Conduct presentations to community groups in accordance with approved communications and marketing plans and state regulations
Requirements
What you’ll need- Bachelor's Degree
- 1+ year experience in corporate social responsibility, community outreach, education, and/or nonprofits
- 1+ year of experience in supporting Medicaid health plans and/or underserved populations
- Familiarity with Florida geography and culture, particularly Northeast Florida
- Must reside in Duval, Clay, Nassau, Baker or St. Johns County
- Proficient in administration and data management systems
- Knowledge of and ability to navigate community resources
- Strong knowledge of community-based and faith-based organization operating structures, social determinates of health and health equity
- Strong presentation skills
- Understanding of marketing, social media and public relations and its connection to a field-based role
- Event, project and/or program management experience
- Minimum download speed of 25 Mbps and upload speed of 10 Mbps for home internet
- Dedicated work-from-home space without ongoing interruptions to protect member PHI/HIPAA information
- Willingness to commute through surrounding counties
- Availability for evenings and weekends as needed
- Ability to accommodate occasional overnights
- Ability to independently set up, assemble, transport, operate, and tear down event materials and equipment weighing up to 20 pounds
