About the role

Key responsibilities & impact
  • Contribute to administration of care management
  • Provide non-clinical support for member needs assessments and evaluations
  • Guide members and families toward appropriate care and wellbeing resources
  • Engage Medicaid enrollees through Humana's Comprehensive Care Support teams
  • Address social determinants of health and support chronic-condition self-management
  • Navigate healthcare systems and promote prevention and tailored health education
  • Conduct in-person assessments of member care needs, preferences, socioeconomic barriers and home environments
  • Assist with scheduling physical and behavioral health visits and address attendance barriers
  • Advocate for members with providers, community resources, schools and other entities
  • Coordinate access to housing, food, income and transportation resources
  • Monitor care-plan adherence and provide motivational interviewing for medication and treatment adherence
  • Provide culturally appropriate social support, health education and health coaching
  • Attend community events to connect with members and educate them about case management services
  • Conduct research and in-person outreach to locate difficult-to-contact members
  • Assist with Health Risk Screenings and Welcome Calls
  • Build relationships with providers and community resources and identify community resource gaps
  • Support Humana's Readmission Prevention and ED Diversion program
  • Collaborate with case managers, housing specialists, SDOH coordinators, ED Liaison and Redetermination Specialist
  • Perform other duties as assigned

Requirements

What you’ll need
  • Must reside in Illinois
  • Minimum two (2) years prior experience working with community resources, community health agencies/social service agencies
  • Valid driver's license, car insurance, and access to an automobile
  • Decision making skills regarding own work approach/priorities, work assignments, standards and resources
  • Ability to multi-task and work in a very fast-paced environment
  • Strong understanding and respect of all cultures and demographic diversity
  • Must be able to work a 40-hour work week, Monday through Friday, 8:00 AM to 5:00 PM; overtime may be requested
  • Must maintain personal vehicle liability insurance meeting state minimum limits or specified Humana limits
  • Home internet with minimum 25 Mbps download and 10 Mbps upload speed
  • Dedicated workspace without ongoing interruptions to protect PHI/HIPAA information
  • Familiarity with state Medicaid program guidelines
  • Experience engaging with Medicaid enrollees, including physical and behavioral health needs and varied health literacy
  • Bachelor's degree in social work or related health field (preferred)
  • Community Health Worker training and/or certificate (preferred)
