About the role
Key responsibilities & impact- Work one-on-one with members to create personalized care plans and coordinate services
- Assess members' goals, needs, barriers, resources, and ability to return to the community
- Provide specialized support to members receiving Long Term Services and Support (LTSS)
- Address health-related social needs and provide psychosocial support
- Support members residing in nursing facilities by building relationships with facility staff and advocating for care
- Contact members telephonically and face-to-face to establish goals, evaluate resources, develop plans of care, and identify providers and community partnerships
- Support members and caregivers in accessing LTSS, social, housing, educational, and other services
- Develop and modify Individual Service Plans and involve relevant care team members
- Navigate LTSS and related environmental and social needs
- Use available member information to prevent duplicative assessments
- Recommend Home and Community-Based Services (HCBS) to support member independence
- Facilitate interactions with payers, providers, and interdisciplinary care teams
- Educate members on maintaining Medicaid eligibility
- Enter annual Level of Care assessments into the state portal
- Travel 75–90% in the field to visit members
Requirements
What you’ll need- Must reside in the state of Illinois
- Bachelor's degree in social sciences, social work, human services, or a related field
- Two (2) years of experience working with Medicare and Medicaid recipients, long-term care services, Home and Community-Based Services (HCBS), or managed care organizations
- One (1) or more years of professional experience working with members in custodial and transitional care
- Experience using electronic information applications/software programs, including electronic medical records
- Ability to travel in the region to meet face-to-face with members, families, community partners, and care teams
- Valid state driver's license
- Personal vehicle liability insurance meeting required limits
- Ability to work Monday–Friday, 8:00 AM–5:00 PM Central Standard Time, with flexibility
- Ability to complete TB screening if selected
