About the role
Key responsibilities & impact- Evaluate members' needs and requirements to achieve or maintain optimal wellness
- Guide members and families toward appropriate care and wellbeing resources
- Visit Medicaid members in homes, assisted living facilities, long-term care facilities, and other care settings
- Travel locally 75–90% of the time
- Assess and evaluate member needs to establish individualized care plans
- Ensure members receive services in the least restrictive setting
- Plan and implement interventions
- Coordinate services and monitor and evaluate case management plans against members' personal goals
- Facilitate interactions with payer sources, providers, interdisciplinary teams, and others involved in members' care
- Report to the Regional Care Coach Manager
- Provide health education, motivation, and encouragement to members
Requirements
What you’ll need- Bachelor's Degree with at least 2 years of relevant experience in Health Services, Social Work, or Psychology; OR Registered Nurse (RN), licensed in the state of Florida, with at least 2 years of relevant experience; OR Licensed Practical Nurse (LPN), licensed in the state of Florida, with at least 4 years of relevant experience; OR Case Worker with a minimum of 6 years of Health Services/Case Management Experience
- Prior experience with Medicare & Medicaid recipients
- Bilingual English/Spanish with ability to pass language proficiency exams in both
- Must reside in one of the listed areas within the region
- Intermediate to advanced computer skills
- Experience with Microsoft Word, Excel, and Outlook
- AHCA Fingerprint Required
- Must complete TB screening if selected
- Valid state driver's license
- Proof of personal vehicle liability insurance
- Ability to work Monday-Friday, 8:30am-5:00pm
- Florida Level 2 background screening through the Care Provider Background Screening Clearinghouse
- Community Health Worker preferred
- Prior nursing home diversion or long-term care case management experience preferred
- Experience with electronic case note documentation and multiple computer applications/systems preferred
- Experience working with geriatric population preferred
- Experience with health promotion, coaching, and wellness preferred
- Knowledge of community health and social service agencies and additional community resources preferred
- 701B Certification preferred
- Medication application (RFA) experience preferred
