About the role

Key responsibilities & impact
  • Evaluate members' needs and requirements to achieve or maintain optimal wellness
  • Guide members and families toward appropriate care and wellbeing resources
  • Visit Medicaid members in homes, Assisted Living Facilities, Long Term Care Facilities, and other care settings
  • Assess and evaluate member needs to establish member-specific care plans
  • Ensure members receive services in the least restrictive setting
  • Plan and implement interventions
  • Coordinate services and monitor and evaluate case management plans against members' personal goals
  • Facilitate interactions with payer sources, providers, interdisciplinary teams, and others involved in members' care
  • Meet members in their locations, assess needs and barriers, and connect them with quality services
  • Report to the Regional Care Coach Manager
  • Travel locally 75-90% of the time

Requirements

What you’ll need
  • Bachelor's Degree with at least 2 years of relevant experience (Health Services, Social Work, Psychology), OR Registered Nurse (RN), licensed in the state of Florida with at least 2 years of relevant experience, OR Licensed Practical Nurse (LPN), licensed in the state of Florida, with at least 4 years of relevant experience, OR Case Worker with a minimum of 6 years of Health Services/Case Management Experience
  • Prior experience with Medicare & Medicaid recipients
  • Must reside in the listed areas within the region
  • Intermediate to advanced computer skills
  • Experience with Microsoft Word, Excel and Outlook
  • AHCA Fingerprint Required
  • Ability to complete Humana's Tuberculosis (TB) screening program
  • Valid state driver's license
  • Proof of personal vehicle liability insurance
  • Ability to work Monday-Friday 8:30am-5:00pm
  • Bilingual English/Spanish preferred, with ability to pass a Language Proficiency Exam
  • Community Health Worker preferred
  • Prior nursing home diversion or long term care case management experience preferred
  • Experience with electronic case note documentation and documenting in multiple computer applications/systems preferred
  • Experience working with geriatric population preferred
  • Experience with health promotion, coaching and wellness preferred
  • Knowledge of community health and social service agencies and additional community resources preferred
  • 701B Certification preferred
  • Medication application (RFA) experience preferred
  • Florida Level 2 background screening through the Care Provider Background Screening Clearinghouse
